Job Summary Physical Therapist (Home Health) Alameda County CA

- Role: Physical Therapist specializing in Home Health
- Location: Sacramento CA (with potential for territory expansion)
- Schedule: 40 hours/week Monday-Friday plus one weekend/month rotation
- Key Responsibilities:
- Prioritize and complete Start of Care (SOC) visits
- Supervise Physical Therapy Assistants (PTAs)
- Submit next-day mandatory reporting for SOC visits
- Complete clinical notes and documentation within 48 hours of each visit
- Maintain timely communication with supervisor intake and office staff
- Remain responsive during normal business hours
- Requirements:
- Valid California Physical Therapist (PT) license
- Minimum one year of Home Health experience
- Productivity:
- 30 points per week (with specific point values per visit type)
- Average caseload and visit mix to total 8 hours/day 40 hours/week
- Overtime requires pre-approval from Clinical Supervisor
- Training:
- 3 full days (1 in-house 2 in the field)
- Documentation System: Kinnser
- Equipment Provided: iPad Verizon One Talk
- Compensation & Benefits:
- Mileage reimbursement
- Parking and tolls reimbursed
- Optional company car (with decreased rate)
