Job Summary:
The Registered Nurse will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients providers and other members of our team.

- Providing skilled nursing care case management and hospice care
- Strong communication and collaboration with patients and providers
Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- BLS certification
Preferred Skills & Certifications:
- ACLS PALS TNCC CNOR certifications
Special Considerations:
- Must be willing to float between like units
- Weekend call shifts required (at least one weekend a month)
- Weekday evening call shifts required (5 evenings per month)
- Must include all RTO at the time of submittal
Scheduling:
- 5x8 Day shifts
- Start date: ASAP
- Weekends Required
- Guaranteed Hours: 70 hours per 2 week period
