About this Position Position/Specialty: Nurse Case Manager (RN) Utilizing a collaborative approach the inpatient case manager will assess plan implement monitor and evaluate the options and services required to meet an individuals health needs. Provides comprehensive on-going case management services to patients by coordinating and managing the care of patients to meet multiple service needs across the continuum of care. Ensure optimal patient outcomes that address quality service customer satisfaction and cost-effectiveness. The Case Manager will partner with the multidisciplinary team to establish care and allocate resources associated with the patients risk assessment. Assists the patient/patients family in coping with illness by optimizing the patients/familys self-care abilities and supporting their consumer rights. Ratios: 1 to 22 Shift: 5x8s 8am-430pm Weekend REQ: (1) 8-hour shift/month Is on-call REQ: No Will clinician float within scope to meet facility needs No Required/Preferred Years of experience REQ: 2 in case management First-time travelers accepted (Y/N): Yes License REQ: SC or Multi State/Compact RN license Certs REQ: BLS Certs Preferred: Case Management certification (CCM) Education REQ: BSN Experience Preferred: Prior leadership experience and Epic Experience Miscellaneous Open to accommodating block schedule Approval prior to assignment but not guaranteed. Radius rule Yes 60 miles. Are locals accepted Yes on a case-by-case basis at 10% less than the posted bill rate Pending License accepted: SC or compact state license in hand at time of start RTO Restrictions: All RTO must be included at time of submission. Approval will be based on facilitys needs. Guaranteed Hours: Facility allowed to call off 1 shift per 2 weeks Hospital Highlights Total Staffed Beds: 97 Scrub Color: Hunter Green Charting: Epic Parking Cost: No Cost **Travelers should be aware that during hurricane season all employees and contractors may be assigned to emergency response teams. If this is a problem please ensure travelers do not confirm a NEW or EXT assignment during the season. Hurricane season runs from June through November each year.
