- Advocate for clients rights and access to necessary resources and services.
- Monitor and review care plans regularly to ensure they meet clients evolving needs.
- Maintain accurate and up-to-date records of client interactions and progress.
- Participate in multidisciplinary team meetings to discuss and address client needs.
- Stay informed about community resources social policies and best practices in social work.
Requirements

- Bachelors degree in Social Work or a related field.
- Proven experience in adult social work or a similar role.
- Strong understanding of social work principles ethics and practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Empathy patience and a genuine desire to help others.
- Familiarity with local community resources and services.
- Valid drivers license and willingness to travel for client visits.
