JOB SUMMARY

The Stroke Program Coordinator is a Registered Nurse who demonstrates advanced knowledge and experience in providing care to the stroke patient. Coordinates and oversees the ongoing process improvement efforts related to the care of the emergent stroke patients within Saint Francis Medical Center.

Responsibilities include:

1. Functions as the Stroke Coordinator
2. Facilitates the annual and monthly community stroke screenings.
3. Offers on-going stroke education opportunities for all levels and departments interacting with the stroke population.
4. Collects and analyzes data regarding the outcomes of the Stroke Program.
5. Submits required data to The Joint Commission and other regulatory agencies as required.
6. Serves as coordinator to ensure compliance with regulatory requirements including the Joint Commission Advanced Primary Stroke Center accreditation and Certificate of Distinction in Disease-Specific Care for Primary Stroke Treatment.
7. Actively works with physicians and other departments to correct identified deficiencies.
8. Maintains clinical knowledge and skills for the stroke population.
9. Serves as a resource to the clinical staff and other departments.
10. Attends and reports to the quarterly stroke team and the Department of Neurosciences meetings disease specific performance measures and other information as pertains to the Stroke Program.
11. Works collaboratively with other members of the healthcare team to ensure patient education initiated completed and comprehended.
12. Establishes and maintains effective relationships with community agencies such as emergency medical services.
13. Works collaboratively with Medical Director of the Stroke Program to ensure practice standards are evidence based and concurrent with the most up-to-date published guidelines.

JOB DETAILS AND REQUIREMENTS

Type: Full Time (80 hours per 2 week pay period with benefits)

Typical hours for this position: Monday-Friday approximately 8:00am-4:30pm

Education:

-Graduate of an accredited school of nursing.

-Bachelors of Science Degree in Nursing from an accredited school of nursing required.

Certification & Licensures:

BLS Certification

Experience:

-Five years clinical nursing experience.

-Three years clinical nursing experience caring for patients in area of specialty.
