Job Summary:
The Registered Nurse Case Manager is responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members are critical to this role.

- Active AK RN License in hand for Auto Offer
- BLS certification
- ACM Certification
- Epic experience
- Acute Care experience
- Clinical Assessment skills
- Medication Administration skills
- Treatment Planning skills
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Active AK RN License in hand for Auto Offer
- BLS certification
- ACM Certification
- Epic experience
- Acute Care experience
- Clinical Assessment skills
- Medication Administration skills
- Treatment Planning skills

- Excellent communication and interpersonal skills

Special Considerations:
- Weekend work is required (every other weekend or 4 weekend days in a 4-week schedule).
- Clinician will float within scope to meet facility needs.
- Only 1 time off request per 13-week assignment of 5 days or less is allowed.
- No holiday RTO for Auto Offer.
- Candidates perm address must be at least 50 miles from facility
- Weekend work is required (every other weekend or 4 weekend days in a 4-week schedule).
- Clinician will float within scope to meet facility needs.
- Only 1 time off request per 13-week assignment of 5 days or less is allowed.

Scheduling:
- 5x8 Days
- Shift times may vary:0830-1700
- Guaranteed Hours: 72 Hours per 2 weeks - Traveler can be called off for 1 8-hour shift per 2 weeks
