Job Summary

Schedule: Monday Friday
Guaranteed Hours: 36 hours per week
We are seeking a dedicated and licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting in Texas. The ideal candidate will have experience working with geriatric populations and be comfortable managing a diverse caseload focused on rehabilitation and functional improvement.
Key Responsibilities-
Evaluate and develop individualized treatment plans for residents

Provide therapeutic interventions to improve mobility strength balance and overall function
-
Collaborate with interdisciplinary team members to ensure comprehensive patient care

Educate patients families and caregivers on treatment plans and home exercise programs

Complete timely and accurate documentation in accordance with facility and regulatory guidelines

Ensure compliance with all state and federal regulations

Active Texas Physical Therapist (PT) license (required)

Minimum of 1 year of experience preferred ideally in Skilled Nursing or Long-Term Care

Strong clinical assessment and care planning skills

Excellent communication and teamwork abilities
