Position Overview

We are seeking a dedicated and licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting in Texas. The ideal candidate will have experience working with geriatric populations and be comfortable managing a diverse caseload focused on rehabilitation and functional improvement.

Key Responsibilities
  • Evaluate and develop individualized treatment plans for residents

  • Provide therapeutic interventions to improve mobility strength balance and overall function

  • Collaborate with interdisciplinary team members to ensure comprehensive patient care

  • Educate patients families and caregivers on treatment plans and home exercise programs

  • Complete timely and accurate documentation in accordance with facility and regulatory guidelines

  • Ensure compliance with all state and federal regulations

Qualifications
  • Active Texas Physical Therapist (PT) license (required)

  • Minimum of 1 year of experience preferred ideally in Skilled Nursing or Long-Term Care

  • Strong clinical assessment and care planning skills

  • Excellent communication and teamwork abilities
