Position Overview

We are seeking a licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) setting. The Physical Therapist will evaluate residents develop individualized care plans and deliver evidence-based therapeutic interventions to improve mobility strength and functional independence.

Key Responsibilities

Perform comprehensive physical therapy evaluations
Develop and implement individualized treatment plans
Provide therapeutic exercises gait training balance training and mobility interventions
Monitor patient progress and adjust treatment plans as needed
Educate patients and caregivers on home exercise programs and safety strategies
Maintain accurate and timely documentation of services provided
Collaborate with an interdisciplinary team to support patient-centered goals
Ensure compliance with facility policies and regulatory standards

Qualifications & Requirements

Active State Physical Therapist license (must be current)
Experience in a Skilled Nursing Facility preferred
Candidates with less than 1 year of experience may be considered
Strong clinical decision-making and documentation skills
Ability to work independently and as part of a rehabilitation team
