Primary Setting: Outpatient Rehabilitation (with some inpatient coverage)
Position Overview We are seeking a dedicated and patient-focused Physical Therapist (PT) to provide high-quality rehabilitation services in an outpatient setting with occasional inpatient coverage as needed. The Physical Therapist will be responsible for evaluating patients developing individualized treatment plans and administering therapeutic interventions to improve mobility function and overall quality of life.
This role involves collaboration with physicians medical staff and interdisciplinary teams to ensure optimal patient outcomes.
Key Responsibilities -
Provide 1:1 patient care with 45-minute appointments scheduled hourly

Perform comprehensive evaluations and appropriate diagnostic testing based on patient diagnosis

Develop individualized treatment plans and measurable goals

Reassess and revise treatment plans every 6 weeks or sooner based on patient progress

Implement appropriate physical therapy techniques and modalities

Maintain timely and accurate documentation to support physician follow-ups insurance authorization and reimbursement requirements

Collaborate with physicians and healthcare team members regarding patient progress
-
Participate in shared inpatient coverage on holidays and occasional Sundays
Qualifications -
Graduate of an accredited Physical Therapy program

Active California Physical Therapist license (must be current)

BLS certification (American Heart Association or American Red Cross)

Minimum of 1 year of PT experience preferred

Strong communication and collaboration skills

Ability to work effectively with patients physicians peers and ancillary departments
