Position Overview

We are seeking an experienced Physical Therapist for a Home Health contract assignment in Solano County CA. The PT will provide skilled therapy services to patients in their homes prioritize start-of-care visits and ensure timely documentation and communication with the clinical team.

Key Responsibilities

Prioritize and complete Start of Care visits
Supervise Physical Therapy Assistants (PTAs)
Submit mandatory next-day reporting for Start of Care visits
Complete clinical documentation within 48 hours of patient visits
Maintain timely and professional communication with supervisors intake and office staff
Remain responsive during standard business hours
Manage an average caseload equivalent to 40 hours per week

Productivity Requirements

30 productivity points per week
Point-to-hour conversion: 1 point 1.33 hours
Average caseload aligns with approximately 8 hours per day / 40 hours per week
Overtime must be pre-approved by the Clinical Supervisor

Visit Point System:

Start of Care 2 points
Recertification / Resumption of Care / OASIS Discharge / Evaluation 1.5 points
Revisit / Non-OASIS Discharge 1 point

Training & Support

3 full days of training (1 in-house 2 in the field)
Documentation system: Kinnser
Equipment provided: iPad and phone system access
Mileage reimbursement (from first to last patient visit)
Parking and toll reimbursements provided
Company car option available

Qualifications

Active California Physical Therapist (PT) License
Minimum of 1 year Home Health experience required
Strong clinical documentation and time management skills
Ability to work independently in a home health setting
