Position Overview

We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) setting. The Physical Therapist will play a key role in evaluating residents developing individualized treatment plans and delivering high-quality rehabilitative care to improve mobility function and overall quality of life.

Responsibilities

Evaluate patients physical conditions and functional limitations
Develop and implement individualized treatment plans
Provide therapeutic exercises mobility training and pain management interventions
Monitor patient progress and modify treatment plans as needed
Maintain accurate and timely clinical documentation
Collaborate with interdisciplinary healthcare teams to ensure continuity of care
Educate patients and caregivers on treatment plans and home exercise programs
Comply with all facility policies procedures and regulatory standards

Requirements

Active State Physical Therapist license (must be current)
Minimum of 1 year of experience as a Physical Therapist
Experience in a Skilled Nursing Facility or rehabilitation setting preferred
Strong clinical communication and documentation skills
Ability to work independently and as part of a multidisciplinary team
