DescriptionPOSITION SUMMARY
This position is responsible for providing direct patient care and organizational leadership to effectively guide the quality of patient care. Provides daily work direction and guidance to department staff and assists with coordinating and evaluation of the activities of the nursing and clinical support staff.
DUTIES AND RESPONSIBILITIES
- Acts as a resource to employees by assisting with coordination of patient care planning and outcomes. Assists with patient scheduling for nursing visits and revisits.
- Schedules regular interdisciplinary team meetings on a weekly basis and assures collaboration between all disciplines to coordinate patient care planning. Educates employees on changes and/or on new material and assists with precepting new hires.
- Reviews quality and operational reports and data with the teams to improve patient financial and clinical outcomes.
- Assumes responsibility for caseloads consisting of patients in geographical/specialty areas. Conducts initial assessment to obtain thorough physical social and environmental data. Establishes the plan of care in accordance with the physician orders and patient needs.
- Provides direct patient care and adjusts nursing care processes as needed. Identifies problems modifies nursing care plan and evaluates outcomes within an appropriate time frame. Advises and consults with family and other personnel as appropriate.
- Provides on-going assessment of patient response to treatments and teaching and applies interventions as appropriate. Updates and revises plan of care as indicated. Maintains and updates accurate clinical and patient records according to agency state and federal guidelines.
- Completes documentation including visit assessment and verbal orders accurately and promptly and within the standards.
- Communicates with physicians to confirm and update patients plan of care. Provides status updates and ensures ongoing communication with the physician and other members of the health care team.
- Facilitates the discharge planning process according to standards and utilizes resources to assure continuity of care after discharge. Participates in developing the discharge plan with the patient/family physician social worker and other staff members as appropriate.
- Participates in chart audits and continuous quality improvement activities as requested. Identifies develops and participates in process improvement opportunities that will enhance the quality of the services provided.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needed as described in the departments policies and procedures. Age-specific information is developed further in the departmental job standards.

QualificationsQUALIFICATIONS
- Demonstrated skill operating in Electronic Medical Records and ability to act as a resource for clinical documentation questions
- Excellent oral written and interpersonal communication skills.
- Demonstrated emotional maturity and ability to cope and work effectively in stressful situations. Demonstrated ability to maintain confidentiality of information.
- Demonstrated ability to work independently with minimal supervision.
- Knowledge of Medicare Medicaid private insurance and other community programs and resources.
- Demonstrated ability to work effectively on an inter-disciplinary team.
- Excellent customer service philosophy and skills required.
- Demonstrated ability to maintain confidentiality of information.
- Demonstrated organizational and time management skills.
- Basic computer skills to include but not limited to Microsoft Word and Outlook required.
- Demonstrated understanding of and concurrence with the hospice philosophy of palliative care.
REQUIREMENTS
- Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
- Ability to operate the necessary equipment to perform the job i.e. FAX computer copier telephone system.
- Graduate of an accredited nursing school required; Possess a current Maryland nursing (RN) license or Licensed MSW. Bachelors degree in Nursing Healthcare Administration or other related field (preferred).
- 2-3 years of nursing experience in a medical/surgical or home care setting required; previous hospice experience strongly preferred.
- Must be able to lift or move patients in a safe manner. Must be able to lift 50 lbs.
- Must have a valid drivers license with a good driving record and a registered functional car with adequate insurance coverage.
- Compliant with health screening and vaccination requirements of organization and position including TB screening and Influenza vaccination annually.
Requirements and Working Conditions Physical
- Must be able to sit stand walk and climb stairs for limited to prolonged periods of time.
- Must have the physical agility to move about in confined spaces including bending twisting kneeling squatting and occasionally reaching one or both over head.
- Must have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest.
- Must be able to concentrate with frequent interruptions.
- Must have accurate vision to read gauges and measuring devices and to interpret written instructions.
