How you will make a difference
As a Case Navigator (Case Manager) you will support clients on a longer-term basis to address the life issues that have caused and perpetuated their homelessness. This will be achieved through housing social engagement education employment skill development and improved wellbeing.
This is a full-time fixed-term role contracted until 30 June 2027
About you
We are seeking applications from experienced case managers who understand the importance of confidentiality and information sharing guidelines. Your ability to build rapport and work in a client-centred way is essential for this role. You must have previous experience in the development and maintenance of case plans and be willing to work in a collaborative multi-disciplinary team. Your high quality client records and case notes will also capture appropriate statistical data for contribution to research needs assessment and data analysis.
Tertiary qualifications in the social services field are essential. Knowledge of or experience in the issues facing people at risk of or experiencing homelessness along with an understanding of homelessness services will set you in good stead to succeed in this role.
You must have or be willing to obtain both a satisfactory National Police Clearance and Working with Children Check. Due to the requirements of the role a valid drivers licence will be required.
