Job Summary

About the role

A Home-Based Care Case Manager supports children and young people living in foster through a therapeutic traumainformed approach that prioritises safety stability and emotional wellbeing. The role provides comprehensive case management including supervising and supporting caregivers completing annual assessments conducting regular home visits and spending meaningful onetoone time with clients with the flexibility to increase contact when risks or needs escalate. It requires strong cultural awareness particularly in relation to Aboriginal children and families along with a solid grounding in traumainformed practice painbased behaviours and social work theory. The Case Manager works collaboratively with caregivers families Child Protection and community services to ensure care plans are coordinated evidenceinformed and responsive to each childs circumstances. Success in the role relies on excellent communication and interpersonal skills the ability to work effectively within multidisciplinary teams while exercising sound judgement independently and a commitment to high standards of documentation legislative compliance and child safeguarding.

This is a Permanent Full-time opportunity based in Geelong within our Home-Based Care Program. The position reports to the Home-Based Care Coordinator

Key Selection Criteria:

To be successful in this role you will have:

  • Knowledge and understanding of Aboriginal culture values and the issues impacting Aboriginal children young people and families.
  • Relevant tertiary qualifications in Social Work Psychology or related behavioural sciences.
  • Strong communication and interpersonal skills with the ability to work collaboratively and exercise sound judgement independently.
  • Demonstrated experience providing case management in complex family situations within the child protection system.
  • Welldeveloped timemanagement and riskassessment skills.
  • Experience in community services such as residential care homebased care disability family services or reunification.
  • Understanding of the Children Youth and Families Act 2005 and child welfare policies.
  • Commitment to traumainformed practice and therapeutic models including the Sanctuary Model.

You must have a:

  • Valid and current Drivers Licence;
  • Satisfactory Criminal History Check (paid for by MacKillop);
  • Valid Employee Working with Children Check;
