In this (full-time max term) Family Integrated Case Management until December 2026 youll work as a part of our 54 Reasons Family & Child Connect team in Mount Isa to help us create lasting change for children everywhere.

You will make a difference by:

  • Building strong trusting relationships with families to identify strengths needs and goals and providing holistic family-centred case management support.
  • Conducting assessments and developing tailored support plans that promote child safety wellbeing and positive family outcomes.
  • Working collaboratively with schools community organisations and service providers to coordinate support and ensure families are connected to the right services.
  • Providing early intervention and practical support to families to reduce risk factors and strengthen family functioning.
  • Maintaining accurate case notes documentation and reporting in line with organisational policies procedures and child safeguarding requirements.

This role requires:

  • Strong expertise in Family Integrated Case Management building trust with families and working collaboratively with schools and community services to create meaningful change.
  • Highly organised reflective and confident in complex situations always maintaining a childcentred and culturally respectful approach.
  • Excellent communication and relationshipbuilding skills enable you to work effectively with families colleagues and external partners to achieve positive outcomes.
  • Relevant tertiary qualification in Social Work Human Services Psychology Community Services or a related field.
  • Demonstrated experience in case management family support or child and family services.
  • A current Working With Children Check (WWCC) and ability to obtain a National Police Check.
  • Strong understanding of child protection child safeguarding and early intervention frameworks.
  • A current drivers licence and willingness to travel locally to support families and engage with schools and community services.
