Are you a compassionate and dedicated Nurse Practitioner ready to make a significant impact in a patient-centered environment Lifestream Health Center a leading provider of pain management and mental health services is seeking a Nurse Practitioner to join our growing team in our Baltimore satellite clinics (Woodlawn Fells Point Parkville Glen Burnie). The Role:
As a Nurse Practitioner in one of our satellite offices you will play a crucial role in delivering high-quality whole-person care to patients experiencing chronic pain depression or substance use disorders. You will manage your clinic with professionalism and autonomy while benefiting from strong support and coordination with our main office. Clinic hours for this role are Monday - Thursday.
Key Responsibilities:
- Perform thorough patient assessments formulate accurate diagnoses and develop individualized treatment plans.
- Manage same-day patient visits and ensure all documentation is completed accurately and timely (within 72 hours).
- Provide in-house treatments such as trigger point injections and pharmacy refills.
- Facilitate appropriate referrals for imaging services Physical Therapy Chiropractic care Orthopedic consultations and laboratory services.
- Coordinate care with the main office to ensure seamless patient transitions and support.
What You Bring:
- Current and unrestricted Nurse Practitioner license in Maryland.
- Active CDS and DEA licenses.
- Experience in pain management mental health behavioral health or addiction medicine is preferred.
- Demonstrated strong clinical judgment excellent time management and meticulous follow-through.
- A patient-first mindset committed to providing compassionate and comprehensive care.
- Comfort and proficiency working independently in a satellite clinic setting with full remote support.
