We are seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join a rehabilitation team supporting patients in a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) setting. This role offers a consistent daytime schedule with guaranteed hours and the opportunity to make a meaningful impact on patient recovery and quality of life.
Schedule:

Day shifts

36 hours per week guaranteed

No holiday coverage required
Responsibilities:

Assist the Occupational Therapist in implementing individualized treatment plans

Provide therapeutic interventions to improve patients functional abilities and independence

Document patient progress and treatment outcomes accurately and timely

Collaborate with interdisciplinary healthcare teams to support patient goals
-
Ensure compliance with facility policies safety standards and regulatory requirements

Requirements:

Active state license as a Certified Occupational Therapy Assistant

Minimum of 1 year of experience in a SNF and/or LTC setting preferred

Strong communication and documentation skills

Ability to work independently and as part of a multidisciplinary team
