Job Summary

We are currently looking for Care Coordination Interns for the Fall 2026 semester!

Want to know more Lets talk about the technicalities! Direct experience opportunities may include but are not limited to:

Shadow Care Coordinator on visits with patients including home visits phone contracts ER visits inpatient mental health visits inpatient substance use visits incarceration visits
Assist with contacting clients currently in outreach to schedule appointments for intakes
Explain qualifications of program and services offered topatients
Assist Care Coordinators in linkage and referral process
Attend community meetings with Care Managers
Assist with administrative tasks as needed

Eligibility Requirements:

Current enrollment in an Associates or Bachelors Level program for Social Work Psychology Mental Health or Substance Abuse Counseling or other related Health and Human Services field required. You are not eligible if you have already graduated from a degree programor if you are not enrolled in the programs listed above. This internship experience must also be a requirement of your degree in order to participateas it is unpaid and for school credit hours only.

Location Information:

This internship is located at 1000 Young Street Tonawanda NY 14150.

Schedule Information:

Our internship supervisors strive to offer flexible schedules; however placements are currently available Monday through Friday. During the application youll be asked to provide your full availability so we can make the best possible match.

Internship Timeline:

This internship is for the Fall 2026 with a start date in August 2026 until December 2026.
Applications will close on March 6th 2026.
Placements will be confirmed by April 6th 2026.
