Job Summary

SH-868 Certified Nursing Assistant (Home Care) New OrleansLA

Position Overview:
Works under the direction of the Home Health Coordinators and reports to the Home Health Manager. This role involves providing participants with assistance in personal care and household management within the home setting. The Certified Nursing Assistant is responsible for maintaining a safe clean and supportive environment while promoting participant independence and dignity.

Key Responsibilities

  • Assist participants with basic activities of daily living including but not limited to bathing dressing grooming incontinent care toileting and other assigned tasks.
  • Assist participants with transfers and mobility with or without assistive devices ensuring safety at all times.
  • Maintain a safe clean and healthy environment through light housekeeping including changing bed linens dusting sweeping cleaning kitchen and bathroom and doing laundry.
  • Provide companionship and stimulation for participants to support emotional well-being.
  • Accompany participants to medical appointments or other outings as needed.
  • Perform housekeeping and personal care tasks as indicated in the participants Care Plan.
  • Remind participants to take medication at the appropriate times as directed.
  • Plan and prepare simple meals; serve meals supplements and snacks; and perform cleanup duties.
  • Assist with oral feeding of participants when necessary following safety protocols.
  • Observe and report any changes in a participants condition to the Home Health Coordinator or Manager in a timely manner.
  • Participate in required training and in-service education programs to maintain and enhance skills.
  • Complete other tasks as assigned by the supervisor.

Qualifications & Requirements

  • Education: High School Diploma or GED preferred.
  • Certifications: Current Certified Nursing Assistant (CNA) License preferred. CPR certification preferred.
  • Experience: Minimum two years of related experience with older adults required.
  • Skills: Strong interpersonal skills with the ability to handle unusual or crisis situations calmly and effectively. Ability to communicate clearly with participants caregivers family members and healthcare professionals.

Working Conditions

  • Perform participant transfers regularly with or without the use of mechanical devices as indicated.
  • Travel to participants homes and visit multiple participants on the same day (typically 4-5 participants).
  • Work with participants with varying cognitive levels physical and sensory impairments limited social supports declining health and at end-of-life.
  • Communicate and interact with participants caregivers and family members on a daily basis.
  • Prepare for the possibility of unexpected crisis situations to occur.

Physical Requirements:
Must be able to lift and carry up to 15 pounds. Position requires standing less than 33% of time on duty walking less than 33% of time on duty and sitting between 33% and 66% of time on duty. Use of hands to finger handle or feel over 66% of time on duty. Reaching with hands or arms less than 33% of time. Full ability to talk and hear. Correctable vision required.
