The Housing and Urban Development VA Supportive Housing (HUD-VASH) Supportive Services for Veteran Families/Progressive Engagement Intake Program Coordinator (SSVFIPC) is responsible for providing coordination of all SSVF intake/ referrals and progressive engagement requests for the HUD-VASH program including case management for a caseload of homeless Veterans in the HUD-VASH program in the PVAHCS catchment area.
Duties include but are not limited to:
1) The incumbent coordinates communication and collaboration with community providers including transitional housing sites and shelters with the goal to provide swift placement into permanent housing for homeless Veterans in the HUD-VASH program.
2) The incumbent performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.
3) The incumbent verifies the Veterans status eligibility for VA medical care and eligibility of program participation.
4) The incumbent is responsible for ensuring that homeless Veterans are considered as primary participants for program acceptance.
5) The incumbent will conduct biopsychosocial assessments to determine needs of Veterans and family members.
6) The incumbent is an active participant in the treatment planning process with other disciplines.
7) The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
8) The incumbent will provide individual family and group counseling as appropriate.
9) The incumbent will provide consultation/education to Veterans and families on community resources Advance Directives and VA benefits.
10) The incumbent will act as a consultant to other team/staff members.
Work Schedule: Monday-Friday 7:30 am to 4:00 pm

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
