- Complete and maintain OASIS documentation in compliance with agency state and federal standards ensuring timely and accurate records.
- Use the nursing process to assess patient and caregiver needs to deliver high-quality evidence-based care.
- Develop communicate and update individualized plans of care in collaboration with patients/caregivers ensuring compliance with regulations and policies.
- Serve as Case Manager for assigned patients actively managing caseloads and visit schedules to meet productivity standards.
- Implement medical plans of care nursing interventions and procedures to ensure safe effective and quality patient outcomes.
- Maintain competency with high-tech nursing skills (e.g. IV therapy wound VAC complex wound care) and adhere to infection control protocols.
- Collaborate with LPNs delegate tasks as appropriate review documentation and ensure care plans are followed.
- Evaluate the effectiveness of care plans by reviewing patient problems interventions and responses to ensure quality outcomes.
- Maintain accurate and current documentation to support effective care coordination within the healthcare team.
- Adhere to agency protocols for McKesson processes including visit completion and documentation requirements.
- Provide comprehensive education to patients and caregivers on medications treatments discharge instructions and available services.
- Identify and document interdisciplinary needs facilitating timely communication and coordination with other healthcare providers.
- Accurately complete necessary agency state and federal forms (e.g. HHABN Medicare Notice of Non-Coverage) during patient visits.
- Conduct visits efficiently coordinate with schedulers and ensure proper visit categorization (e.g. supervisory vs. routine).
- Assess patient needs for HCA services develop and supervise implementation of plans of care with ongoing oversight.
- Submit incident reports (e.g. infections injuries) to Quality Improvement Specialist as needed and in a timely manner.
- Coordinate safe patient discharges and transfers ensuring all required documentation and information is processed appropriately.
- Collaborate with physicians providers team members and vendors to support and facilitate comprehensive patient care.
- Participate in monthly peer review audits giving and accepting feedback professionally.
- Proactively schedule and facilitate case conferences ensuring timely and effective team communication and documentation.
- Advocate for patient care issues ensuring care is tailored and sensitive to the unique needs of each patient and family.
- Provide hospice coverage as required when on-call.
- Work schedule: Sunday - Thursday 8am-4pm with some on-call responsibilities.
