JOB PURPOSE:The Home Study and Post-Release Services (HSPRS) Case ManagerLevel 2willprovidecase management and supportive services to unaccompanied immigrant children released from federal HSPRS Case Manager will work in collaboration with program staff and supervisors to cultivate a client-centered and trauma-informed approach to service provisions.



Qualifications

ESSENTIALJOBRESPONSIBILITIES:

  • Conduct home visits and phone contacts with the dual purpose of assessing safety andappropriateness ofchildsplacement with their caregiver (sponsor) and connecting childto services in the community to address risk factors.
  • Ongoing case management with a focus on screening and makingappropriate referralstoaddress childs needs in the following domains: Placement issues safety issueseducation health mental health legal/immigration proceedings substance abuseindependent living criminal/juvenile justice child enrichment activities.
  • Conduct age-appropriate comprehensive assessment in achild centered trauma-informed manner and documented inan online case management system.
  • Provide crisis intervention services directly as needed or connect family to services toensureminorssafety and well-being ina crisis situation.
  • Activelyestablishenhanceandmaintainliaison relationships with key governmentagencies and non-profits/ community organizations.
  • Comply withpolicies and procedures governing the program and ensure that the programisoperatingat maximum efficiency.
  • Meet or exceed program productivity expectations on a regular and sustained basis.
  • Other duties as assigned.

ESSENTIALQUALIFICATIONS:

  • Minimum of 1 years of experience relevant to the position and working with vulnerable or migrant populations; 3 years preferred.
  • Proficiencyin Spanish/English required(written and spoken language skills).
  • Excellent verbal and written communication skills.
  • Ability to handle confidential material in a responsible manner.
  • Able and willing to travel domestically up to 50% oftime.
  • Proficiencyin Microsoft Office applications.

EDUCATION:Bachelors degree in the education psychology sociology or other behavioral sciences equivalent.

EXPERIENCE:SeeEssential Qualifications and Education.

ATTENDANCE:Mustmaintainregular and acceptable attendance at such level as isdeterminedin the employers sole discretion.

LICENSES:DriversLicense withclear record.

VEHICLE:Must have daily use of a vehicle without prior notice.

OTHER:Must be available and willing to travel to various locations and with such frequency as the businessneeddictates.
