Job Summary:
The Registered Nurse is responsible for delivering skilled nursing care case management and hospice care to individuals experiencing aging frailty and serious illness. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Effective communication and collaboration with patients providers and team members are essential.

Responsibilities:
- Conduct patient assessments to determine care needs.
- Administer medications as prescribed.
- Develop and implement patient treatment plans.
- Collaborate with an interdisciplinary team to coordinate patient care.
- Provide skilled nursing care in various settings including medical/surgical ICU ER psychiatric labor & delivery PACU and operating room.
- Provide case management services.
- Provide hospice care to patients and their families.
- Maintain accurate and timely documentation of patient care.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Drivers license car insurance and automobile for field work.
- Minimum of 2 years of hospital home health or hospice care experience.
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
Special Considerations:
- May involve working in a fast-paced environment.
Scheduling:
- Full-time position.
