ABOUT THE POSITION





The County of Riversides - Riverside University Health System (RUHS) UR- Case Management divisionis seeking a qualified Licensed Vocational Nurse IIto join their team located in Moreno Valley.

Under supervision and direction of a registered nurse nurse practitioner physician assistant or physician performs nursing duties within the scope of the Nurse Practice Act for vocational nursing in the care of patients in the Department of Mental Health or Public Health Ambulatory Family Health Center; and perform other related duties as required.

The most competitive candidate will have experience in Case Management with Utilization Management experience preferred. This position supports patient care through effective case management and discharge planning. The incumbent will plan and coordinate patient discharges to home or appropriate post-acute care facilities beginning the discharge planning process at the time of admission. Responsibilities include assessing discharge needs ensuring discharge plans are clear thorough and timely and recommending appropriate durable medical equipment and support services. The role requires knowledge of community resources and contracted facilities as well as the ability to arrange follow-up care and facilitate referrals to community providers and vendors. The incumbent will also educate patients families caregivers and community partners on post-acute care plans to support safe and successful transitions of care.

The Licensed Vocational Nurse (LVN) is not an independent practitioner. Incumbents assigned to the LVN II class have one to three years of experience working as an LVN.

This is the full journey level class for LVNs working in non-acute care assignments with the Department of Mental Health or Public Health Ambulatory Care. Incumbents assigned to this class perform the full range of professional nursing duties within the scope of the Nurse Practice Act for vocational nursing with a minimum of supervision and periodic in-service training. LVN II incumbents may compete for promotion to LVN III at the end of four years of experience and demonstrate proficiency in independent practice as an LVN within the scope of the Nurse Practice Act for vocational nursing. The LVN II is distinguished from the class of Nursing Assistant Health Services Assistant or Certified Medical Assistant in that a state license is required of incumbents and they perform a more highly skilled form of patient care.

Required valid licenses and certificates:
  • California Vocational Nurses License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners
  • Basic Life Support (BLS) certification from the American Heart Association

EXAMPLES OF ESSENTIAL DUTIES

  • Plans and coordinates patient discharges to home or the appropriate facility. Begins to identify discharge needs upon admission. Ensures that the discharge plan is thorough clear and comprehensive.
  • Determines the appropriate post hospital discharge destination. Understands Community resources and the different contracted facilities. Assesses discharge needs and recommends the appropriate durable medical equipment and other support in a timely manner.
  • Provides thorough documentation for the medical record the patient and caregivers. Maintains a complete and accurate file of appropriate community-based services and facilities.
  • Applies evidence-based review criteria tool (like McKesson/Interqual) to expedite discharges and recommend DME and other necessary supplies.
  • Arranges follow up needs and facilitates referrals when necessary.
  • Knowledge of appropriate community services and facilities that can meet the patients post-discharge clinical and social needs; such as Skilled Nursing Facilities; long-term acute care rehabilitation services Home Health care Hospice or other appropriate levels of care) to which the patient can be transferred or referred.
  • Ability to provide the patient and family/caregivers with accurate information and written and verbal instructions in preparation for the patients after-acute care needs.
  • Ability to determine whether the patient is capable of addressing his or her care needs through self-care; whether family or friends are available who are willing and able to provide the required care at the times needed or who you could train to sufficiently provide such care.
  • Knowledgeable to educate the patient family/caregivers and community providers about the patients after-acute care hospital/post-acute care facility care plans.
  • Ability to implement a complete timely and accurate discharge plan. Ability to identify high risk criteria and elevate concerns and involve the appropriate team members when issues and concerns arise.

MINIMUM QUALIFICATIONS

Experience: At least one year of successful experience as an LVN preferably in a public health clinic environment.

Knowledge of: Current principles techniques and procedures used in vocational nursing within the scope of the Nurse Practice Act for vocational nursing; medical terminology acute care procedures and equipment; medication administration and any policies and procedures of the setting assigned.

Ability to: Skillfully perform the full range of vocational nursing duties and responsibilities within the scope of the Nurse Practice Act for vocational nursing; learn and follow policies and procedures for the setting assigned; establish and maintain effective working relationships with physicians mid-level providers registered nurses patients and co-workers; complete and maintain reports and patient records; make observations of situations accurately and report them so effective action can be taken according to rules of documentation and policies and procedures.


Other Requirements:
License: Possession of a valid California Vocational Nurses License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Possession of a valid Basic Life Support (BLS) certification issued by the American Heart Association for professional healthcare providers.

A valid California Drivers License is required for some positions in this class.
