Under the direction of the Program Manager the Drop-In Center Case Manager provides high-quality center-based case management services to individuals and families accessing services through the Drop-In Center. The Case Manager serves as a primary point of contact for participants supporting stabilization service linkage and long-term self-sufficiency through structured on-site support services.
Key Responsibilities:
- Conduct client intakes and comprehensive needs assessments at the Drop-In Center
- Complete psychosocial assessments and develop individualized service plans in collaboration with participants
- Provide on-site case management services in a welcoming trauma-informed environment
- Assess client needs and connect participants to appropriate community resources and referrals
- Provide resource navigation and service coordination including housing referrals healthcare access benefits enrollment employment resources and social services
- Maintain accurate and timely documentation case notes and client records
- Build trusting professional relationships with participants
- Support clients in achieving stability goals related to housing health income and self-sufficiency
- Coordinate care with internal staff and external service providers
- Utilize harm reduction strengths-based and client-centered approaches
- Manage an active caseload of participants receiving Drop-In Center services
This position is fully center-based. Services are provided on-site at the Drop-In Center only. No field work outreach or home visits are required.
Requirements
Qualifications:
- Knowledge of homelessness poverty and social service systems
- Strong interpersonal organizational and communication skills
- Ability to work with diverse populations with complex needs
- Experience conducting intakes assessments and service planning
- Familiarity with Los Angeles County social services and community resources
- Trauma-informed culturally responsive and client-centered approach to care
- Ability to work in a fast-paced high-volume service environment
- Flexible schedule including some evenings and weekends as needed
- Experience with case management documentation systems preferred
Required Skills:
Minimum Qualifications: * Degree/Education: Bachelors of Science in Nursing. Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE) or the Commission on Graduates of Foreign Nursing Schools (CGFNS). * Experience: Minimum of three (3) years experience as a clinical nurse in the medical/surgical or critical care inpatient setting. Experience with newborn to geriatric population. Minimum of 1 year of experience in post anesthesia/sedation. * Licensure: Current full active and unrestricted license to practice as a Registered Nurse from any state. * Life Support Certification:Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or Advanced Life Support (ALS) Pediatric Advanced Life Support (PALS) and Trauma Nursing Core Course (TNCC) certifications are required. * Security: Must possess ability to pass a Government background check/security clearance.
