Base salary range: $125000 $130000 (flexible depending on experience)

Responsibilities

Monday Friday 8AM 5PM
No nights or weekends
Minimal call responsibility (1 week 34 times per year; triaged through call center)
Deliver comprehensive value-based primary care to a geriatric patient population
Manage small patient panels (1216 patients/day) in an outpatient setting
Conduct in-clinic care with occasional home visits and telehealth appointments
Perform physical exams assessments and diagnostic testing
Create and implement individualized care plans focused on chronic disease management
Educate patients and families on medication nutrition prevention and disease progression
Collaborate with a multidisciplinary team including MA social worker pharmacist and specialists
Document care accurately to support risk adjustment and quality metrics
Prescribe medications within state scope of practice

Requirements

Masters degree in Nursing
Board certification as a Nurse Practitioner (Adult Adult-Gero or Family)
Minimum of 2 years outpatient primary care experience
Autonomous practice capability required
Experience with value-based care preferred
Strong background in geriatrics and chronic disease management
Proficiency with EMRs (Athena preferred)
Must hold an active NP license in the state of California and DEA certification (or be eligible)
Spanish speaking required
Experience in wound care ABIs injections or imaging interpretation preferred
