Position Summary
The Director of Individual Service Plans & Outcomes Management oversees the full process used to develop document track and evaluate goals for individuals served by the agency. This role ensures that all programs use consistent standards accurate data and reliable workflows when creating and monitoring special-needsrelated goals. The Director manages Therap integration maintains system integrity and works closely with program staff to ensure that goals are measurable updated appropriately and aligned with each individuals needs. The position also leads the agencys approach to reporting outcomes and supporting continuous quality improvement across all programs.
