Employment Type: Full-Time exempt

Reports To:Chief Operations Officer

Position Summary

The Director of Individual Service Plans & Outcomes Management oversees the full process used to develop document track and evaluate goals for individuals served by the agency. This role ensures that all programs use consistent standards accurate data and reliable workflows when creating and monitoring special-needsrelated goals. The Director manages Therap integration maintains system integrity and works closely with program staff to ensure that goals are measurable updated appropriately and aligned with each individuals needs. The position also leads the agencys approach to reporting outcomes and supporting continuous quality improvement across all programs.

Core Responsibilities

  1. Goals Facilitation Tracking & Therap Integration

Goal Development & Data Integrity managed through Therap

  • Lead the agency-wide process for facilitating Special Needs goals and documentation in Therap
  • Establish clear workflows for assessments interdisciplinary input and family/guardian collaboration
  • Develop clear standards for what qualifies as a good goal.
  • Ensure goals are in-place measurable assessment-aligned and consistently developed
  • Train and support staff in the standardized facilitation of goals setting and data tracking
  • Build systems that support reliable progress monitoring without direct auditing
  • Serve as the agencys subject-matter expert for Special Needs components in Therap

2. Outcomes & Achievement Framework

  • Develop the framework for measuring and reporting goal outcomes
  • Guide programs in determining goal achievement and needed revisions
  • Set quarterly and annual reporting expectations
  • Support creation of goals that drive meaningful progress for individuals served

3. System Integrity & Quality Assurance

  • Ensure workflows templates and staff practices produce consistent high-quality documentation
  • Maintain integrity of consumer planning data through uniform standards
  • Serve as the steward of the Special Needs planning system ensuring compliance and efficiency
