Job Summary
The Home Health Manager (RN) plays a vital leadership role in a Program of All-Inclusive Care for the Elderly (PACE) environment helping frail and vulnerable seniors remain safely in their homes and communities. This role is accountable for high-quality home-based clinical care as well as oversight of services delivered in nursing homes and board-and-care settings. The Home Health Manager arranges and supervises nursing physical therapy and occupational therapy ensuring that participants receive coordinated value-based care aligned with their individualized care plans.

Essential Job Duties

  • Direct a high-performing home care department ensuring clinical workflows policies and procedures are consistently followed.

  • Lead guide and support home care staff including hiring onboarding training coaching and performance feedback.

  • Assess the effectiveness of home health nursing assessments and participation in the interdisciplinary team (IDT).

  • Ensure home care nurses deliver coordinate and document nursing interventions as outlined in participants care plans.

  • Oversee the quality of care provided to participants in nursing homes and board-and-care facilities including collaboration with external providers.

  • Promote a culture of safety quality regulatory compliance and patient-centered care for a frail elderly population.

Job Requirements

  • Current California Registered Nurse (RN) license required.

  • Bachelors Degree in Nursing or a related field preferred; additional years of relevant experience may substitute for formal education.

  • Minimum 2 years of nursing experience in a home health setting preferably working with frail or elderly participants.

  • Knowledge of geriatric nursing and the ability to manage complex clinical scenarios in the home and facility settings.

  • Strong leadership communication and interdisciplinary collaboration skills.
