Job Summary
The Home Health Manager (RN) plays a vital leadership role in a Program of All-Inclusive Care for the Elderly (PACE) environment helping frail and vulnerable seniors remain safely in their homes and communities. This role is accountable for high-quality home-based clinical care as well as oversight of services delivered in nursing homes and board-and-care settings. The Home Health Manager arranges and supervises nursing physical therapy and occupational therapy ensuring that participants receive coordinated value-based care aligned with their individualized care plans.
Essential Job Duties

Direct a high-performing home care department ensuring clinical workflows policies and procedures are consistently followed.

Lead guide and support home care staff including hiring onboarding training coaching and performance feedback.

Assess the effectiveness of home health nursing assessments and participation in the interdisciplinary team (IDT).

Ensure home care nurses deliver coordinate and document nursing interventions as outlined in participants care plans.

Oversee the quality of care provided to participants in nursing homes and board-and-care facilities including collaboration with external providers.

Promote a culture of safety quality regulatory compliance and patient-centered care for a frail elderly population.
Job Requirements

Current California Registered Nurse (RN) license required.

Bachelors Degree in Nursing or a related field preferred; additional years of relevant experience may substitute for formal education.

Minimum 2 years of nursing experience in a home health setting preferably working with frail or elderly participants.

Knowledge of geriatric nursing and the ability to manage complex clinical scenarios in the home and facility settings.

Strong leadership communication and interdisciplinary collaboration skills.
