Job Summary

Position Title: Clinical - LTSS Service Care Manager

Location: Edinburg Edcouch McAllen Mission TX

Schedule: Mon Fri; 8-5pm No OT allowed

Duration: 12 Months Contract

Job Description:

Must be available to work 8am-5pm

Assist in biannual assessments in person & option for telehealth if member is in agreement and has capabilities.

Complete assigned contractual outreach calls and document findings.

Coordination with multi-disciplinary teams.

Follow up calls to members.

Any other projects that may arise.

Must be able to meet strict turn-around times with documentation for assessments assigned.

Monthly outreach log provided and expectation is to complete all by the end of the month

Candidate Requirements

Education/Licensure Required: Licensed Practical/Vocational Nurse (LPN/LVN)

Years of experience required:

preferred at least 2yrs care coordination

Disqualifiers:

No car no internet service poor computer skills; vacation needed first 2 months of hire

Additional qualities to look for:

basic computer skills needed. Needs to be able to navigate MS Office Suite
