This role focuses on care coordination and discharge planning in a fast-paced Emergency Department to promote timely transitions and efficient patient flow.

Responsibilities

Coordinate patient care across ED services
Conduct utilization reviews and discharge planning
Collaborate with interdisciplinary teams
Identify discharge needs and arrange services
Ensure regulatory and payer compliance

Qualifications

NY State RN license
5 years clinical RN experience
Care management and UR experience
ASN/ADN required
BLS required

Preferred

BSN or higher

Prior care management leadership
