Position SummaryTrainee Aged Care and Disability Services
The traineeship is a fixed term role with a scope for extension
The Trainee will work across the Aged Care and Disability Services with a primary focus on engaging members of the Aboriginal Community who are eligible for support both in their homes and the wider community.
Support Workers are responsible for the frontline delivery of services to individuals or groups providing practical assistance that enables clients to maintain independence wellbeing and social connection.
This role may include:
- Supporting clients to maintain independence in their own homes.
- Assisting with recreational and leisure activities.
- Providing homebased support duties including but not limited to:
- Vacuuming
- Dusting
- Sweeping
- Mopping
- Washing and changing bed linen
Key Responsibilities
Care Planning and Administration- Assist individuals to develop maintain and achieve their goals and independence in activities of daily living.
- Complete accurate and timely case notes and client records as required.
- Maintain daily weekly and monthly documentation within data management systems for both planned and unplanned service delivery.
- Undertake other duties as required within the scope of the role.
Relationship Management- Foster and maintain positive relationships with other agencies to benefit clients and the organisation.
- Attend relevant meetings forums and education programs to ensure contemporary practices research and approaches are integrated into MDAS service delivery.
- Work collaboratively with internal teams to ensure streamlined intake assessment and referral processes.
SelfManagement and Team Contribution- Maintain confidentiality in line with the MDAS Code of Conduct and organisational policies and procedures.
- Report through appropriate escalation pathways any concerns that may place a client or their family at unacceptable risk.
- Contribute to the development and improvement of policies and procedures supporting continuous improvement.
- Work in accordance with all MDAS corporate policies and State and Commonwealth guidelines.
Corporate Responsibilities- Support values that respect historical and contemporary Aboriginal cultures recognising the special connection Aboriginal people have with the State.
- Comply with relevant Quality Risk Management and Occupational Health and Safety legislation standards and safe work practices.
- Participate in required training and professional development opportunities.
- Actively monitor and contribute to improvements in the quality and safety of care and services.
- Assist with and support internal and external audit and accreditation processes as required.
Key Selection CriteriaService Excellence- Ability to work efficiently and effectively while taking direction from senior staff.
- Ability to exercise discretion at all times and maintain confidentiality.
Communication and Interpersonal Skills- Strong verbal and written communication skills.
- Ability to interface and coordinate work effectively with both internal and external stakeholders.
Problem Solving- Ability to solve problems using knowledge experience and guidance from senior staff.
Organising and Planning- Strong organisational and coordination skills.
- Basic administrative skills and ability to manage tasks and resources within required timeframes.
Respect for Cultural Diversity- Demonstrated willingness to provide inclusive services to Aboriginal people.
- Commitment to learning about other cultures to strengthen relationships and improve service delivery.
Qualifications- Willingness to undertake Certificate IV in Home and Community Care or Aged Care (as required by MDAS).
Other Mandatory Screening Requirements- Current Australian Drivers Licence.
- Current Employee Working with Children Check (Victoria).
- Current National Police Check.
