The primary goal of the Adult Protective Services program is to provide timely and effective intervention for at-risk adults living within the community. We aim to prevent harm and foster an environment of safety and security through comprehensive assessment and intervention tailored to the individual needs.
RESPONSIBILITIES
- Provide direct services to clients including intake engagement assessment serviceplandevelopmentreferraladvocacycounselingexitsummariesand follow-up.
- Manageacaseloadandmonitoranddocumentclientprogresstowardservice plan goals.
- Monitorriskandassessthestatusofcurrentserviceplansaswellasupdate/ revise the service plans as the clients needs change
- Assesandreferclientstoappropriateserviceswhichincludebutlimitedto:Deep cleaning home health services legal services mental/ medical care
- Utilizemotivationalinterviewinginordertocompleteassessmentsandservice plans
- Ensurethatservicesplansarecompletedwithin30daysandserviceplans reviews are completed every 6 month
- Maintainfamiliaritywithprogramresourcesavailableon-andoff-site.
- Travelall5boroughsinNYC.
- Coordinateclientserviceswithothersocialservicementalhealthmedical employment educational child care and other providers.
- Conducthomevisitsasrequiredbycontractandweeklytelephonic communication.
- Completeprogramreportsonamonthlybasisormorefrequentlyasrequired.
- Monitorriskandassessthestatusofcurrentserviceplansaswellasupdate/ revise the service plans as the clients needs change.
- Seekappropriateguidancefromthesupervisorinsituationsrequiring clarification and consultation.
- Reviewandsignoffonserviceplansandpsycho-socialassessmentsasassigned.
- Reporttoandmeetwithsupervisoronamonthlybasis.
- Completeinitialhomevisitswithin24-48hoursofassignment
- Documentallcommunicationwithclientswithin24hoursinETOdatabase
- comfortable with operating agency vehicle
- Perform additional duties as assigned.
QUALIFICATIONS
- Bachelorsdegreeinsocialworkorotherrelatedfieldsofstudy.
- 4-yearsofexperienceincasemanagementorinanotherrelatedfieldofwork.
- Certifiedchildabuseandneglectmandatedreporterorabilitytoobtain certification within 3 months of employment.
- EnrollmentinMSWprogrampreferred.
- Possessacleanvaliddriverslicenseandbewillingtodriveacompanyvehicle.
- Engagingpersonalityanddetailedorientatedskillsareessential.
- Strongoralandwrittencommunicationtimemanagementandorganizational skills are necessary.
- Proficiency in Microsoft Office suite and other standard business technology is required.
