Job Summary

We are looking for compassionate and skilled individuals who are committed to making a positive impact in the lives of the most vulnerable adults. Together we can create a safer and more supportive environment for those in need. The primary goal of the Adult Protective Services program is to provide timely and effective intervention for at-risk adults living within the community. We aim to prevent harm and foster an environment of safety and security through comprehensive assessment and intervention tailored to the individual needs.


RESPONSIBILITIES

  • Provide direct services to clients including intake engagement assessment serviceplandevelopmentreferraladvocacycounselingexitsummariesand follow-up.
  • Manageacaseloadandmonitoranddocumentclientprogresstowardservice plan goals.
  • Monitorriskandassessthestatusofcurrentserviceplansaswellasupdate/ revise the service plans as the clients needs change
  • Assesandreferclientstoappropriateserviceswhichincludebutlimitedto:Deep cleaning home health services legal services mental/ medical care
  • Utilizemotivationalinterviewinginordertocompleteassessmentsandservice plans
  • Ensurethatservicesplansarecompletedwithin30daysandserviceplans reviews are completed every 6 month
  • Maintainfamiliaritywithprogramresourcesavailableon-andoff-site.
  • Travelall5boroughsinNYC.
  • Coordinateclientserviceswithothersocialservicementalhealthmedical employment educational child care and other providers.
  • Conducthomevisitsasrequiredbycontractandweeklytelephonic communication.
  • Completeprogramreportsonamonthlybasisormorefrequentlyasrequired.
  • Monitorriskandassessthestatusofcurrentserviceplansaswellasupdate/ revise the service plans as the clients needs change.
  • Seekappropriateguidancefromthesupervisorinsituationsrequiring clarification and consultation.
  • Reviewandsignoffonserviceplansandpsycho-socialassessmentsasassigned.
  • Reporttoandmeetwithsupervisoronamonthlybasis.
  • Completeinitialhomevisitswithin24-48hoursofassignment
  • Documentallcommunicationwithclientswithin24hoursinETOdatabase
  • comfortable with operating agency vehicle
  • Perform additional duties as assigned.


QUALIFICATIONS

  • Bachelorsdegreeinsocialworkorotherrelatedfieldsofstudy.
  • 4-yearsofexperienceincasemanagementorinanotherrelatedfieldofwork.
  • Certifiedchildabuseandneglectmandatedreporterorabilitytoobtain certification within 3 months of employment.
  • EnrollmentinMSWprogrampreferred.
  • Possessacleanvaliddriverslicenseandbewillingtodriveacompanyvehicle.
  • Engagingpersonalityanddetailedorientatedskillsareessential.
  • Strongoralandwrittencommunicationtimemanagementandorganizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.
