Travel RN - Hospice Case Manager
Barre VT
Start Date: 03/30/2026
Shift: Monday-Friday 40 hrs/week (Onsite)
Weekly Pay: $2584
Job Description
Seeking an experienced Hospice RN Case Manager to provide compassionate end-of-life care in a home health setting. This role requires strong clinical judgment excellent communication skills and the ability to manage patient care independently.
Key Responsibilities

Provide hospice nursing care to patients in their homes

Manage and coordinate hospice care plans for assigned patients

Conduct patient assessments and monitor symptom management
-
Collaborate with physicians and interdisciplinary care teams
-
Educate patients and families on hospice care and end-of-life support

Document patient care accurately in the EMR system
Qualifications
-
Current Vermont RN license (full unrestricted)
-
Hospice case management experience required
-
Experience providing hospice care in a home setting required

Experience with McKesson/Netsmart EMR preferred
-
Prior experience working in rural travel settings preferred

Strong communication organization and critical thinking skills
