Job Summary
Job Title: AK - Registered Nurse Case Manager
Location: Juneau AK
Start Date: ASAP
Shift: M-F - Days 40 hrs/week
Weekly Pay: $3536
Job Description:
MUST HAVE ACTIVE ALASKA LICENSE at time of submittal.
Case Management experience must be within the last 2 years.
PROOF OF FLU VACCINE REQUIRED.
Fingerprint cards required for onboarding. Start date will be minimum 5 weeks from date fingerprint cards are received by SEARHC.
Key Responsibilities:

Hybrid/blended role: manage SEARHC patients AND serve as clinic nurse as assigned.

Collaborate with medical providers and interdisciplinary team members to establish individualized patient care goals.

Coordinate multi-disciplinary care conferences for high-risk or complex patients.

Manage acute and chronic care needs health maintenance and outreach/education via EHR.

Participate in case management reviews and act as patient advocate.

Facilitate patient referrals within SEARHC and track outcomes.

Perform staff nursing duties: assessment documentation education and administering medications/therapies.

Maintain competencies infection control and safe work environment.

Participate in triage and emergency situations as needed.

Perform other duties as assigned.
Qualifications:
-
Graduate of accredited school of nursing; BSN preferred.

Current unrestricted RN license in Alaska.
-
Basic Life Support (BLS) certification.
-
National Case Management certification preferred.

High school diploma or equivalent required.
Experience Required:

At least 1 year of case management experience within the last 2 years.
