Job Summary

We are seeking a dedicated Wisconsin Licensed LPN to join our resident care team. This role is perfect for a nurse who enjoys the home-like environment of a CBRF while providing professional nursing oversight. You will be responsible for health monitoring medication management and supervising resident care staff to ensure the highest quality of life for our residents.

Primary Responsibilities

  • Clinical Oversight: Perform health monitoring wound care and nursing interventions as prescribed.
  • Medication Management: Oversee the administration and storage of medications in compliance with WI DHS 83.
  • Leadership: Supervise and mentor Resident Assistants and CNAs ensuring safe patient handling and adherence to care plans.
  • Documentation: Maintain accurate clinical records and update Individual Service Plans (ISPs).
  • Flexibility: Work collaboratively across morning afternoon or night shifts based on resident needs and your schedule.

Wisconsin State-Specific Requirements

Compliance with WI DHS 12 and DHS 83 is a condition of employment:

  • Licensure: Must hold a current valid Wisconsin LPN License.
    • Verification will be conducted via the Wisconsin DSPS / Nursys online portal.
  • Background Check Compliance: * WI Background Disclosure Form (F-82064): Must be completed and signed before any background checks are initiated.
    • WI Caregiver Background Check: Required at hire and every 4 years thereafter.
    • Out-of-State/National Checks: Required if you have lived outside of Wisconsin in the past 7 years.
  • CBRF-Specific Training:
    • Exemptions: As a Licensed LPN in Wisconsin you are exempt from the Big Four CBRF classes (Medication Administration Standard Precautions First Aid/Choking and Fire Safety) provided your license is in good standing.
    • Required Training: You must complete Disability Support Assistant Training and Safe Patient Handling / Back Safety training upon hire.
  • Physical Requirements: Ability to perform safe patient transfers and lift/move up to e.g. 50 lbs using proper ergonomic techniques.

Qualifications

  • Graduate of an accredited Practical Nursing program.
  • Active Wisconsin LPN License.
  • Current CPR/BLS certification.
  • Familiarity with Wisconsin DHS 83 regulations (LTC/CBRF) is highly preferred.
  • Strong communication skills and a passion for working with the elderly or those with disabilities.
