Job Responsibilities (Duties and Responsibilities include but are not limited to):
1.Complete a timely and comprehensive initial home health physical therapy assessment with history of current and previous illness identifying acute and chronic conditions.
2.Use standardized testing to measure patients initial and subsequent physical performance while establishing and adapting individualized goals to optimize clients performance.
3. Accurately document the level of patients disability due to co-morbidities and or other disease process and rationalize frequency/duration of skilled care referring services to appropriate discipline as necessary.
4.Skillfully analyze patients performance and adapt plan to reach comparable performance norms research based peer level performance and/or correctly identify plateau in performance. Differentiate and deliver improvement based services or maintenance therapy programs as clinically appropriate. We will teach you the difference between Restorative and Maintenance Therapy.
5.Develop and implement appropriate skilled and sophisticated Rehabilitation program to improve and/or maintain the patients current condition while preventing or slowing down further deterioration under maintenance standard.
6.Identify discharge planning needs as part of care plan development and implement prior to discharge of the patient.
7.Act as Case Manager when assigned by Clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload.
