Job Summary RN Case Manager Home Health/Hospice - Assess screen evaluate re-evaluate and treat home health and hospice patients.
- Develop individualized care plans in collaboration with patient family and physician.
- Periodically review and update patient care plans including recertification as needed.
- Coordinate patient care through interdisciplinary communication and case conferences.
- Provide timely communication regarding patient discharges to appropriate team members.
- Perform ongoing assessments of patients clinical functional and emotional status.
- Communicate significant patient status changes to physicians and care team.
- Document all evaluations treatment goals and objectives accurately.
- Educate patients and caregivers on self-care techniques medications diets and health counseling.
- Deliver care with compassion integrity and effectiveness in a home health/hospice setting.
- Work independently while also collaborating with a cohesive patient-care team.
- Manage a flexible schedule to support work/life balance.
- Serve communities in the Vermont and New Hampshire area covering over 70 towns.
Qualifications: - Licensed Registered Nurse in Vermont and New Hampshire (or compact license).
- Graduate of an accredited nursing program.
- Minimum 2 years of experience in Home Health and Hospice.
- Current certifications (e.g. ACLS BLS/BCLS PALS etc.) as required.
