Job Description
- Perform in-home nursing assessments (medical functional psychosocial environmental)
- Develop personalized care plans to promote independence
- Coordinate services with healthcare providers and community resources
- Advocate for participant rights and self-determination
- Monitor ongoing care and adjust plans as needed
- Maintain accurate documentation in EMR systems
- Collaborate with families clinicians and support planners
- Determine visit frequency based on participant need
