ABOUT THE ROLE
Role: Aged Care Connector Elder Care Support (ACC)
This is an opportunity for a fixed term until June 2026 - with exciting potential for extension!

The ACC has primary responsibility for connecting Elders community members and older Aboriginal and Torres Strait Islander people and families to aged care entitlements and supports.

The role ensures support is organised sequenced and appropriate to enable access to aged care services internally and externally.

The ACC works closely with the Aged Care Support Coordinator and assists in identifying local demand for aged care services and any service gaps.

Advocacy is also a key part of the role.

KEY RESPONSIBITIES
Community Engagement and Direct Support
  • Active community outreach to promote aged care services and engage potential clients.
  • Support clients to identify their goals and needs to ensure appropriate levels of care.
  • Provide advocacy and support through the My Aged Care registration and review process.
  • Support clients to attend appointments.
  • Work with the aged care team to review and enhance service delivery.
  • Work with internal and external stakeholders to evaluate the service.
  • Ensure materials developed meet the needs of Aboriginal and Torres Strait Islander communities.
  • Support clients and families to select appropriate aged care services and providers.
  • Support families regarding client care needs including linking to other services transfers of care and providing information about care options and costs.
Consumer and Carer Participation
  • Support clients in planning with aged care providers to ensure cultural care and safety needs are met.
  • Deliver services in a culturally competent manner.
Relationship Management
  • Foster and maintain relationships with agencies to benefit clients and the organisation.
  • Attend meetings and forums to remain up to date with developments research and models of care.
  • Work collaboratively with internal partners for streamlined intake assessment and referral processes.
Corporate Responsibilities
  • Support values that recognise Aboriginal peoples special connection to the State.
  • Comply with Quality Risk and OHS legislation policies and safe work practices.
  • Actively monitor and improve the quality and safety of care and services.
  • Assist with internal and external audit processes.
Administration
  • Maintain accurate case notes assessments data entry and reporting requirements.
  • Maintain safe work practices in accordance with OHS policies.
  • Ensure adherence to MDAS policies procedures legislation and accreditation standards.
Organisational Responsibilities
  • Participate in service reviews audits and development activities.
  • Ensure programs reflect MDAS core values.
  • Review community needs to ensure effective service delivery.
  • Contribute to ongoing development of quality systems and standards.
Team Effectiveness
  • Build effective relationships with partner organisations.
  • Work collaboratively as part of the team.
  • Support shared accountability and knowledge sharing.
  • Cooperate with team members to ensure continuity of care.
  • Participate in meetings planning PDR supervision and staff development.

Stakeholder Engagement
  • Promote programs and develop linkages with Aboriginal community agencies and mainstream services.
  • Participate in network meetings as required.
  • Ensure promotional materials are distributed in the community.
  • Support the quality team in developing resources and training packages.
  • Participate in community education relating to accessing NDIS and understanding the scheme.
  • Represent MDAS to raise awareness of aged care services and promote the organisation.
KEY SELECTION CRITERIA
Community
  • Strong relationships with Aboriginal and Torres Strait Islander communities.
  • Experience in aged care or community services.
  • Experience in community development education and capacity building with Indigenous communities.
Stakeholder
  • Demonstrated ability to engage community stakeholders in planning delivery and evaluation of services.
  • Experience working collaboratively with communities and partners.
Verbal & Written Communication
  • Welldeveloped verbal and written communication skills including the ability to produce accurate reports.
Knowledge
  • Understanding and commitment to Aboriginal health and wellbeing.
  • Knowledge of aged care frameworks.
  • Understanding of personcentred care and traumainformed practice.

  • Commitment to providing culturally inclusive services and building cultural knowledge.

QUALIFICATIONS

  • Certificate in Community Services/Aged Care or willingness to obtain within 12 months (training provided).
  • Current First Aid Certificate or willingness to obtain

  • National Police Check
