What Youll Do

As a Private Duty RN Case Manager you will:

• Assess clients to determine eligibility and appropriateness for private duty home care

• Complete admissions and develop individualized plans of care

• Collaborate with physicians caregivers and referral sources to ensure seamless care transitions

• Coordinate services and resources to support continuity of care

• Partner with schedulers to ensure appropriate caregiver placement

• Supervise RNs LPNs and Home Health Aides to ensure quality care and accurate documentation

• Review clinical documentation to ensure alignment with the plan of care

What Youll Need

• Graduate of an accredited Registered Nursing program

• Active RN license in the state

• 2 years of clinical or home health experience preferred

• Strong communication and organizational skills

• Commitment to high-quality patient-centered care

• Ability to work collaboratively within an interdisciplinary team

• Two weeks of full-time availability required for orientation
