Job Summary

Job Summary:

The Health Information Tech I position is responsible for processing health information under the direction of the HIM Director or supervisor. Key functions include coordinating physician medical record completion and performing quantitative analysis of medical records based on established standards. The role requires knowledge of medical record documentation physician chart completion and proficiency in basic computer skills.

Responsibilities:

- Safeguards and preserves the confidentiality of patients protected health information.
- Notifies physicians of medical records requiring completion and maintains documentation of notifications.
- Administers medical staff guidelines related to medical record completion consistently.
- Retrieves incomplete records and assists physicians in completing records electronically.
- Activates temporary suspension of medical staff privileges for non-compliance.
- Analyzes and re-analyzes incomplete medical records to ensure completeness.
- Scans loose filing into the ChartMaxx System.
- Utilizes ChartMaxx for deficiency analysis and reporting.
- Oversees all incomplete medical record activities and functions.
- Assists physicians with record completion issues and escalates them if needed.
- Completes RLDatix Incident Reports for potential compensable events.
- Conducts record review functions with established criteria and provides data.
- Performs basic eScription monitoring pending and look-up functions.
- Operates standard office equipment including computers photocopiers and scanners.
- Answers unit phone and directs calls accordingly.
- Performs data entry into electronic medical database systems.
- Documents patient care and related duties appropriately.
- Performs selected patient registration activities.
- Coordinates equipment/supply needs and notifies appropriate departments of malfunctions.
Required Skills & Certifications:

- Knowledge of Windows Software
- 3-5 years of Medical Record experience in an acute care setting
- Knowledge of Medical Record documents
- Physician chart completion and chart deficiency analysis skills
- Basic keyboarding skills (35 wpm)
- Knowledge of medical terminology and familiarity with computers
- Ability to categorize forms/documents within the medical record
- Knowledge of Title 22 CIHQ Conditions of Participation Medical Staff Bylaws and Medical Staff Rules and Regulations
Preferred Skills & Certifications:

- High School graduate or equivalent
- Knowledge of physician record completion and HIPAA

- Ability to withstand the pressure of continual deadlines and receipt of work with variable requirements
- Ability to concentrate and maintain accuracy despite frequent interruptions
- Ability to be courteous tactful and cooperative throughout the workday
- Must be detailed oriented and self-motivated
