The HACC-PYP Coordinator is responsible for coordinating services under the Home and Community Care Program for Younger People (HACC-PYP) which supports Victorians under 65 (or under 50 for Aboriginal and Torres Strait Islander people) who are at risk of losing independence due to disability chronic illness mental health or other conditions.
This role ensures participants receive timely person-centred and goal-oriented support that promotes independence wellbeing and community participation. The HACC-PYP Coordinator upholds the principles of wellness reablement and restorative care while meeting program guidelines and compliance requirements.

This is a part-time (0.6 FTE) fixed term contract until 30 June 2028. Full-time would be considered for the right candidate.
The role can be based from our Echuca Bendigo or Maryborough office.
The role is classified under the Social Community Home Care and Disability Services Industry Award Level 4 with an hourly starting salary of $44.58 plus superannuation and access to salary packaging benefits.
