The Bowery Mission is a Christian faith-based organization committed since the 1870s to serving people experiencing homelessness hunger and poverty. The Bowery Mission meets essential needs and creates transformative communities with men women children and families in New York so that we can together overcome homelessness and poverty and flourish.
As a key member of the Tribeca Campus Choose Help team the Intake Coordinator develops integrated faith-based clinical support that motivates and equips shelter guests to obtain quality holistic care and achieve goals. Supports Choose Help Social Workers through care coordination and short-term case-management. Works cohesively with Ambassadors to connect guests to viable resources. Completes referrals to services that support an effective and comprehensive internal and external continuum of care. Leads wellness and life-skill group activities at Day Center that facilitate guests motivation toward a better quality of life. Provides conflict resolution and de-escalation interventions using a holistic Christ-centered approach. Participates in regular program team meetings. Develops collaborative cross-department/campus and community partnerships to coordinate care and enhance services. Promptly and accurately records guest progress and programming information.
Specific responsibilities include but are not limited to:
- Build durable and supportive relationships to facilitate guests internal motivation for change and ability to attain goals.
- Connect guests to internal resources such as TBMs Residential Program and viable external resources.
- Manage a caseload in addition to providing general support to guests.
- Meet regularly with guests on caseload to review their Needs Assessments make connections to internal and external resources and follow-up on referrals made.
- Assist in scheduling appointments identifying transportation needs and prioritizing their efforts.
- Facilitate and/or coordinate Day Center groups/classes on topics such as emotional wellness goal setting etc.
- Collaborate with Choose Help staff in support of key program functions such as front desk operations Day Center activities guest check-in and orientation and inclement weather response.
- Ensure all guest data is promptly and accurately recorded. Keep attendance records for classes/groups. Uphold a high standard of data integrity. Follow incident reporting protocol.
Schedule: Tuesday-Saturday: 10:00am-6:30pm
Requirements:
- Embraces the organizations Statement of Faith.
- 2 years experience in a relevant setting (ministry non-profit or human services) with support to managers/specialists.
- Bachelors degree in Social Work Psychology Human Services or other relevant field or equivalent work and education.
- Case-management fundamentals.
- Administrative skills.
- Proficient in Office technology.
- Confidentiality standards.
Preferred:
- Certified inMental Health First Aid.
- Certified in Non-violent Crisis Intervention.
- Motivational Interviewing.
- Trauma-informed Care.
- First Aid/CPR.
- Naloxone Administration.
Salary: $24.04 per hour
