Position Overview:
Under direct supervision of the Intake Manager will be responsible for the receipt assessment verification and data entry of all incoming disease reports. This includes all required reportable diseases other disease reports electronic medical and lab records and addition the individual will determine the jurisdiction of cases and make referrals to out-of-jurisdiction (OOJ) using the transfer protocol. The Intake Coordinator will be expected to possess strong attention to detail collaborate effectively with team members and demonstrate the capability to conduct initial assessments and promptly respond to inquiries. Additionally the role necessitates strict adherence to established protocols and a comprehensive understanding of procedures and documentation standards.

Duties & Responsibilities:
  • Patient Coordination: Coordinates receipt assessment and distribution of cases reported by mail fax e-fax telephone and electronic lab reports (ELRs) from the Texas Department of State Health Services (DSHS) Infectious Disease Control Unit (IDCU). Also reviews and assesses disease reports and documents for appropriate processing data entry into various surveillance systems and electronic medical records database.
  • Collaboration: Determines the jurisdiction of cases. Collaborates with local and out-of-state health departments on referring non-Harris County jurisdiction reports including International jurisdictional (IJN) to other public health jurisdictions using the surveillance priority transfer protocol.
  • Documentation: Scans and documents all incoming medical records and laboratory reports about investigations encompassing but not limited to laboratory reports and medical records.
  • Reporting: Maintains and updates Excel spreadsheets daily offering weekly status reports. Conducts quality assurance for data entry into electronic medical records ensuring accurate and current case registry information. Additionally it ensures timely uploading of case reports to the Department of State Health Services (DSHS).

Requirements

Education:

  • High School Diploma/GED
Experience:
  • At least two (2) years of experience performing clerical office duties in a health care setting (hospital physician or dental office WIC clinic) documentation in and maintenance of confidential records utilization of medical and disease terminology frequent multi-tasking activities.
Knowledge Skills and Abilities:
  • Proficient in MS Office Suite (Word Excel PowerPoint Outlook).
  • Excellent communication skills (verbal & written).
  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment.
  • Ability to maintain confidentiality when dealing with sensitive information.
  • Experience working with electronic medical record software.

General Information

Position Type and Typical Hours of Work:

  • This is a full-time grant-funded position.
  • Hours of Monday - Friday from 8:00 am to 5:00 pm.
  • Occasional weekends holidays and overtime may be required based on the specific needs and demands of the department.
Work Environment:
  • This position is based in an office environment where the use of standard office equipment such as computers phones scanners and filing cabinets is routine. The role is predominantly sedentary involving mainly sitting with occasional standing and walking.
Physical Demands:
  • The physical demands associated with this job are minimal with occasional lifting and moving of objects weighing up to 20 pounds.
