About the Role:
The Senior Case Manager leads a team in providing Rapid Rehousing and Prevention Services. The Senior Case Manager will provide support to the case managers and support the first housing model with a trauma-informed care approach that will maximize the likelihood of client success and long-term self-sufficiency. The Senior Case Manager will collaborate with the Director to support case managers in ensuring services are implemented with a trauma-informed strength-based and client-centered approach. This position will provide services to a reduced caseload in addition to providing staff supervision.
How you will contribute:
Duties and Responsibilities:
- Provide supervision to Case Manager(s) providing Rapid Rehousing and Prevention Services.
- Provide coaching and support to case management team through regularly scheduled one on one meetings and evaluations
- Support Case Managers in delivering complex case management for cases that demand higher level solutions
- Collaborate with the Director to ensure consistent service delivery that is in line with grant and contract requirements company policy and procedure and best practices
- Disseminate information to related to tenant rights and responsibilities housing discrimination communicating with landlords basic unit maintenance lease compliance and housing search and location techniques
- Oversee HMIS data entry compliance
- Coordinate and manage all referrals to ensure timely and accurate service delivery.
- Review and respond to referrals from households experiencing or at risk of homelessness.
- Develop maintain and share a comprehensive resource library and related communication materials.
- Promote program awareness and build trust with community partners to enhance engagement.
- Assist clients in completing required documentation and paperwork throughout the program process to ensure timely access to services and housing.
- Provide housing navigation support including identifying available housing options facilitating landlord engagement and assisting with the lease-up process.
- Meet regularly with assigned households to provide individualized case management services and referrals that promote housing stability.
- Ensure timely communication and information-sharing with all relevant partners and stakeholders.
- Support clients in creating and maintaining household budgets and developing strong money management skills.
- Participate in collaborative meetings with representatives from faith-based community-based and partner organizations to deliver coordinated wraparound services.
- Help clients overcome common barriers to securing housing such as large rent arrears low credit history and misunderstandings of landlordtenant rights.
- Assist clients in achieving long-term housing stability by ensuring compliance with lease agreements and developing Individualized Housing Sustainability Plans.
- Mediate conflicts involving households roommates or landlord-tenant issues that may threaten housing stability.
- Serve as the primary point of contact for landlords and foster positive ongoing relationships.
- Develop and maintain a comprehensive Resource Directory of local and state service providers.
- Maintain accurate thorough and timely case documentation in HMIS and client files in accordance with agency standards and best practices.
- Collaborate as a contributing member of a multidisciplinary team with Shelter House staff and community partners.

RequirementsRequired:
- A bachelors degree in human services / related field or commensurate experience
- 3 years of human services or direct services experience
- Ability to prioritize competing priorities and make sound judgements
- Ability to advocate assist in problem-solving and achieve results for families
- Strong written and oral communication skills
- Strong organizational and time management skills
- Valid Drivers License and reliable transportation
- Flexibility to work outside of normal business hours when needed
Physical Requirements:
- Annual TB Test is required
- Ability to sit or stand for long periods
- Ability to lift items weighing 10-20 pounds
