Berry Streets Transitional Youth Support Services (TYSS) supports young people aged 1625 who are experiencing homeless or at risk of homelessness through assertive outreach housing support and early intervention.
Within TYSS the Homelessness Intervention and Prevention (HIP) stream provides early intervention for young people aged 1618 at risk of homelessness due to family conflict or breakdown.
As a Case Manager you will deliver housing-focused case management outreach and early intervention to support young people and families to achieve safe stable housing build independent living skills and strengthen connections to family community and support networks.

You are passionate about working alongside young people to support safety stability independence and hope. You bring strong case management skills an understanding of youth homelessness and family systems and the ability to engage young people in complex and often stressful circumstances.
