About the Project:

Despite multiple digital health initiatives primary healthcare systems often remain fragmented due to siloed purpose-specific applications leading to inefficiencies poor accountability and gaps in continuity of care. PATH is supporting the Government of Maharashtra to digitize primary healthcare delivery in select districts through an end-to-end saturation model with a focus on RMNCH outcomes. The initiative spans rural and urban areas public and private sectors and aims to build a digitally enabled health workforce interoperable systems longitudinal patient records and data-driven decision-making contributing to improved service coverage quality of care and progress towards Universal Health Coverage.

PATH is currently seeking a Senior Consultant to assist the Government of Maharashtra in measures for revamping the primary health care in accordance with the IPHS Standards and enabling them to provide comprehensive primary care services. This is in accordance with Ayushman Bharat vision. The Consultant should have substantial experience in digital primary health care. Diversity of experience will be appreciated.


The components of the Consultancy assignment will be:

  • An analysis of primary health care service delivery across the State of Maharashtra by studying available data and information and assessing gaps.

  • Field visits to districts to vet the available data for finalizing the above analysis

  • Completing broad analysis in three months from time of assignment

  • Studying good practices for primary health care revamping including use of technology as appropriate

  • Assessing gap areas

  • Helping the Government identify the action points for addressing gaps in primary health care delivery

  • Prepare a road map for addressing those gaps

  • Assisting the government and stakeholders in addressing the gaps

  • Continuous field visit to understand guide and provide feedback on work being done


The work will entail assistance towards:

  • Possible standardization of primary health care organizations in terms of infrastructure service models facility design and core service packages

  • Use of appropriate technology such as digital health platform with integrated analytics

  • Strengthening referral mechanisms to establish continuum of care

  • Promoting use of telemedicine initiatives

  • HR capacity building in key domains includingFamily Medicine Geriatric Care Palliative and Pain Management Mental Health andNCD Care. PromotingMedical Officers as Community Health Managers and Leaders enabling a more participatory community-oriented and culturally contextual primary care approach.

  • Design and implement structured continuing medical education (CME) programs. Establish career progression pathways for primary care physicians based on qualifications experience clinical competencies and health outcomes delivered.

  • Task transference and skill mix strategiesin rural settings to optimize primary care workforce deployment.

  • Establish systematic feedback loops for patients and community stakeholders to continually monitor and improve the quality of primary care services.

  • Develop a comprehensive framework aligningclinical operational infrastructural digital and human resource componentsof primary care delivery.

  • Undertake detailed budgeting and financial planning to ensure the sustainability of the revamped primary care model including cost-effectiveness assessments and funding models.

Education Qualification/Required Skills & Experience:

  • 15 years of experience and demonstrated skills in public health program and and government facility transformation projects (experience in digital health projects preferable)

  • Experience of working with state level government health department is desirable

  • Doctorate in public health or similar fields from a reputable graduate school

  • Technical knowledge and field experience of planning and implementation of public health programs and health systems strengthening in Maharashtra especially RMNCH.

  • Familiarity with Indian public health system flagship initiatives in health sector and relevant government policies/ strategies particularly NHM Ayushman Bharat Health & Wellness Centers ABDM National Health Programs etc.
