Progeny HR Solutions LLC is seeking a highly qualified and experienced Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) to join our team as a Manager of Case Management in Savannah GA. As a full-time employee you will play a crucial role in overseeing and managing the case management process for our clients. Responsibilities: - Oversee and manage the case management process for clients ensuring timely and effective communication with all parties involved - Conduct initial assessments and develop individualized care plans for clients - Collaborate with healthcare providers and other professionals to coordinate and facilitate the delivery of quality care - Monitor and evaluate the progress of clients and make necessary adjustments to care plans - Provide support and guidance to case managers and assist in resolving any issues that may arise - Ensure compliance with all relevant laws regulations and company policies - Maintain accurate and up-to-date records and documentation - Participate in team meetings and provide input to improve processes and procedures - Act as a liaison between clients families and healthcare providers to ensure a smooth and efficient case management process Requirements: - Current and valid RN or LCSW license in the state of Georgia - Minimum of 3-5 years of experience in case management preferably in a healthcare setting - Strong leadership and communication skills - Excellent organizational and time management abilities - Ability to work independently and as part of a team - Proficient in computer skills and electronic medical records - Knowledge of relevant laws regulations and policies related to case management - Compassionate and empathetic approach to working with clients and their families - Willingness to travel within the Savannah area as needed We offer a competitive salary and benefits package as well as opportunities for professional growth and development.
