The Opportunity

The IPC Health Seniors Lifestyle Victoria Program

The opportunities are Full Time or Part Time Fixed Term roles from January 2026 until June 2028 and is classified as a Grade 2 under the Victorian Stand Alone Community Health Centres Allied Health Professionals Enterprise Agreement 2022-2026.

The Seniors Lifestyle Victoria program offers access to a range of services to support participants to stay independent and well. This includes support with physical activity nutrition medication management and social connection.

You will make a difference by

• Supporting older people to remain active connected and independent in their community by acting as a primary point of contact working collaboratively with participants their families and multidisciplinary teams.
• Develop and implement tailored action plans withclients presenting with complex needs that address their goals and promote sustainable improvements in health and wellbeing.
• Apply depth of knowledge to coordinate access to health and social care systems facilitating access to appropriate services and empowering participants to take an active role in their own health journey.
• Proactively collaborating across internal and external partners to promote service enhancement and learning through quality improvement.
• Supporting wellbeing navigators and the team administrator in delivering the Seniors Lifestyle program.

To succeed you will need:

• A tertiary qualification and 3 years experience in allied health or related health field.
• Excellent holistic clinical assessment and care coordination skills.
• Passion for supporting older persons to build knowledge skills and confidence to self manage their health.
• Demonstrated experience in stakeholder engagement with proven ability to strategically engage negotiate and influence others to achieve better outcomes.
• Experience empowering adults in a planned and structured way to improve health and wellbeing outcomes particularly with diverse and disadvantaged communities.
• Strong technical skills with ability to adapt to new platforms and client management systems.
• Current Victorian drivers licence and Working with Childrens Check.
