We are looking for someone who wants to help people with disabilities and the elderly obtain appropriate quality services to maximize independence dignity and quality of life and works with aging and advocates for people with disabilities interested parties community partners WA State Developmental Disabilities Council The Arc Washington and Area Agencies on Aging to ensure a client-focused service delivery system.

Join the Home and Community Living Administration (HCLA) and help support both DDCS and HCLA to develop short-term housing resources for clients displaced from their home for short duration needs such as home modifications adaptations or pest eradication. This position is part of a team assigned responsibility for development of partnerships services subsidies and resources to increase availability stability and access to affordable and accessible housing and tenancy support for individuals with complex needs.

As a Social and Health Program Consultant 4 youll serve as a subject matter expert and trusted resource for staff and leadership. Youll provide consultation training technical assistance and policy interpretation while helping ensure program compliance quality assurance and continuous improvement. Youll also collaborate with internal teams community partners and advocates to strengthen service delivery for clients statewide.

This position plays a critical role in translating complex policies and systems into clear usable guidance for staffand bridging our services in an one of a kind role.

• 40 hours per week

• Hybrid/Remote: Flexible hybrid work available anywhere within Washington State
• Official Duty Station: Lacey WA
• Travel: Occasional travel may be required based on business needs

Some of What Youll Do

• Develop housing services including HRSN rent/temporary housing and resources necessary for successful transition and sustainability in living in community settings for clients leaving nursing facilities and other institutional residential or unstable settings.
• Supporting HCS and DDCS field staff in policy driven authorization and documentation of benefits in Care Web including periodic case audits.
• Promoting awareness of HRSN rent/temporary housing and ability to leverage for temporary dislocation client needs to HCS and DDCS field staff.
• Working in cross-sector partnerships with Home and Community Living Administration (HCLA) HRSN Unit other state agencies and community providers to integrate resources with HRSN related programs.
• Work with Spokane Housing Authority Community Choice Guides and Supportive Housing Providers to implement and develop HRSN rent/temporary housing across HCS and DDCS for temporary displacement needs.
• Collaborating with DDCS to create processes for eligible DDCS clients to receive HCS MIST housing.
• Providing training and technical assistance to support provider capacity development and provision of quality services for HCS and DDCS clients.

• Review case records to ensure accurate assessments and documentation
• Communicate policy procedure and program updates to staff
• Compile data and produce reports using a variety of tools as requested
• Develop and maintain performance tracking and quality assurance tools
• Conduct quality assurance reviews identify gaps and address training needs
• Participate in meetings outreach efforts and travel as needed

Who Should Apply

You qualify if you meet one of the following:

Experience in a related field of work may substitute year for year for the required education.

• One year as a Social and Health Program Consultant 3 (or equivalent) or
• Two years as a Social and Health Program Consultant 2 (or equivalent) or

• A masters degree in social work health or social science public administration or related field and four years of relevant professional experience or
• A bachelors degree in social work health or social science public administration or related field and five years of relevant professional experience in planning administering developing or delivering social financial health or chemical dependency treatment services programs.

Note: Social Work degrees must be from programs accredited by the Council on Social Work Education.

• Understanding of and commitment to the principles of person-centered planning consumer choice and self-direction.

• Demonstrated skills in community development and projectplanning.
• Demonstrated ability to collect and analyze data from systems and make thoughtful recommendations that result in positive program changes. Compiling data extracting information and generating comprehensive reports.
• Work collaboratively in a multi-disciplinaryteam.
• Excellent communication skills ability to write and speak concisely to partners and agencies both public and private from diverse populations.
• Demonstrated ability to develop positive working relationships with a broad range of entities.
• Demonstrated ability to coordinate multiple activities and timelines creating qualitydeliverables.
• Demonstrated ability to work independently plan organize and produce quality deliverables in multiple assignments time frames
• Demonstrated proficiency in computer and software use including DataMart Microsoft Word Excel Outlook PowerBI and PowerPoint as well as SharePoint SharePoint Lists CARE system and SSRS Reports.
